@elaterite @cvvhrn @ai6yr I still use the Deepwater Horizon as a case study in one of my classes. It's a juicy accident with lots of human failures so it plays well with students.

Oh yeah as we talk about when we do a root cause analysis at the hospital its usually a Swiss Cheese Error Model
The problem with these post reviews is few businesses really stick to the "you will not get into trouble for being honest" and if its a system error versus a personal that person will be punished for thier honesty.
After seeing it a few times most will simply not bother as there is too much risk
