It really has a lot of hallmarks of a phishing scam.

Addressed directly to me. Applies on me financial responsibility. Using language of urgency to take action. No prior communication of this responsibility.

But it has very specific details that would only be available if they had a data breach.

But it appears to be an actual bill. I've gone to the Quest website and it's a valid bill.
